Boone Health home care closed—what Columbia families can do now

A practical, respectful guide for Columbia families who still find old Boone Health home care pages while searching for skilled home health.

Boone Health ended its home care and hospice services in December 2023. Old location pages can still appear in search results, which may confuse families who are trying to arrange care after a hospital stay.

If you were given an old phone number or found a page that does not clearly state whether skilled home health is still available, return to the current discharge list or ask the ordering provider for active agency choices.

You still have a choice

The hospital or physician may provide a list, but the patient can choose among qualified agencies that cover the address, can provide the ordered services, and can accept the referral.

Verify the basics

Look for a current office address, local phone, fax, hours, service-area list, and direct Medicare Care Compare record. Avoid sending patient information through a generic contact form that does not explain how it protects clinical details.
