This page explains what the public website and its online forms handle. It is separate from the privacy notices that apply to clinical records.

What this policy covers

This policy applies when you visit orthohh.com or use a form linked or embedded here. It does not replace OHH's clinical Notice of Privacy Practices or describe every right that may apply to a patient record. Contact the office if you need information about the privacy of an OHH clinical record.

Information you choose to send

The callback form asks for your name, phone number, county, and an optional general question. Do not enter health or insurance information there. Healthcare providers may send clinical information through the secure referral form or by fax.

OHH-managed Google Workspace forms

The callback, referral, and satisfaction-survey forms embedded on this website submit directly to OHH-managed Google Workspace forms. Responses are stored in restricted records available only to authorized OHH personnel. The website's analytics code does not receive the answers entered inside these forms.

The separate Google-review invitation is optional and public. Please do not include medical details in a public review.

How submitted information is handled

OHH uses the information you send to respond, evaluate service, and coordinate requested care. If a form is unavailable, use the direct form link or call or fax OHH instead.

First-party site measurement

When you first arrive, the site stores the landing page, referring site, campaign labels, and first-visit time in your browser's local storage. This record remains in that browser until its site data is cleared. Do not place a name, diagnosis, insurance number, or other private information in a campaign link.

If you tap a phone link or request Google Maps directions, the site may send the event name, page path, and first-visit attribution to OHH's first-party event endpoint. It stores the event only when OHH's reporting service is configured. It does not receive form answers.

Google Analytics

This site uses Google Analytics to understand page visits and improve the public website. Its standard measurement may collect the page viewed, page title, referring site, session statistics, approximate location, and general browser, device, language, and screen information. Google Analytics uses first-party cookies to distinguish visitors and sessions.

OHH's website tag denies advertising storage, advertising user data, and ad personalization, and disables Google advertising signals. It removes URL query strings from page locations and reduces an outside referrer to the referring site's origin before sending measurement to Google Analytics. OHH does not intentionally send names, phone numbers, form answers, diagnoses, or insurance information to Google Analytics.

Learn how Google uses information from sites that use its services. You may block or delete cookies in your browser or install the Google Analytics opt-out browser add-on. The site's basic information remains available if analytics is blocked, although third-party forms may follow Google's own settings.

Hosting, service providers, and other websites

Cloudflare hosts and secures the public website and may process standard technical connection information needed to deliver it. Google provides Analytics and the Workspace forms. OHH may also use service providers to operate the site, meet legal duties, and support requested care under applicable agreements.

Links to Medicare Care Compare, Google Maps, Facebook, Google reviews, email, and other websites are governed by those services' privacy practices after you leave this site.

Sharing, retention, and safeguards

OHH does not sell form information. Form responses, website events, and analytics are kept according to OHH's account settings, operational needs, and applicable recordkeeping duties. OHH uses administrative and technical safeguards appropriate to the information, but no internet transmission or storage system can be guaranteed completely secure.

Your choices and questions

You may call instead of using a form. You may clear this site's cookies and local storage through your browser and use Google's opt-out tool for Google Analytics. For website privacy questions or requests, email patientcare@orthohh.com or call (573) 474-1786 during Monday–Friday 8:00am–5:00pm.

Changes to this policy

OHH may update this page when the website, its service providers, or applicable requirements change. The date below shows the latest revision.
