Discharge instructions meet real life at the front door

At home, “walk regularly” becomes a hallway, a bathroom trip, and three steps at the entrance. Medication instructions become bottles on the kitchen counter. The Home Stretch connects the provider's order to those real rooms, routines, equipment, and questions.

Who may qualify

  • Patients coming home after knee or hip replacement.
  • Patients following an ordered recovery plan after a fracture or spine procedure.
  • Patients who need intermittent skilled nursing or therapy and for whom leaving home requires help, equipment, or a major effort.

Eligibility, coverage, the service address, current need, and OHH's ability to staff the referral must be confirmed before admission.

The care plan works toward the ordered goals and the next appropriate setting, such as outpatient therapy or independent work.

One coordinated home health team

Depending on the order and patient need, care may include Skilled Nursing, Physical Therapy, Occupational Therapy, Speech Therapy, Medical Social Work, and Home Health Aides. Medical Social Work and Home Health Aides are currently unavailable. Our clinical team reviews patient outcomes and continuously refines our therapy protocols.
