OHH nursing and therapy help eligible fracture and spine patients follow the ordered plan in their actual home setup.
Precautions are not one-size-fits-all.
The location of a fracture, the type of procedure, weight-bearing status, brace instructions, and spine precautions can all change the plan. OHH clinicians follow the physician's order and teach the patient-specific instructions.
What the team may address
- Safer walking, transfers, balance, and equipment use.
- Everyday activities while following ordered movement precautions.
- Skilled assessment, medication teaching, and ordered incision or wound care.
- Caregiver teaching and coordination when the home setup makes the plan harder.
Start with the actual home.
Entrances, stairs, sleeping arrangements, bathrooms, and support from family all affect recovery. Home health allows the team to see those conditions and practice the plan where it must work.
Know the boundaries.
Home health is intermittent skilled care, not 24-hour supervision. Before discharge, ask who to call for routine questions, changes in condition, and emergencies.
