The care plan should work in your home—not only on paper.
OHH clinicians help eligible patients recover, manage new medical needs, move more safely, and handle everyday routines at home. Your provider's order determines which services are part of the plan.
Know what you are choosing.
Home health is ordered, intermittent clinical care delivered through planned visits. It may help an eligible patient recover, learn a treatment plan, move more safely, or manage daily activities.
Home care usually means non-medical help such as companionship, housekeeping, or ongoing personal assistance. Medicare home health does not provide round-the-clock care.
If you are not sure which kind of help your family needs, call. We will explain the difference without pressure.
