The surgeon's plan comes first.
No two knees, homes, or recoveries are identical. OHH clinicians follow the order and plan of care, then connect it to the patient's real walking paths, chair heights, bathroom, steps, and support at home.
What care may include
- Skilled assessment, medication teaching, and ordered wound care.
- Practice with walking, transfers, balance, strength, and the assigned home exercise plan.
- Safer ways to bathe, dress, use the bathroom, and manage everyday routines.
- Teaching for the patient and family about the plan and which changes to report.

When to ask for OHH
You can ask the ordering provider or discharge planner to consider The Home Stretch before leaving the hospital or surgery center. Eligibility, staffing, coverage, and the service address must still be confirmed.
